Every year the attached forms MUST be completed for EACH child that will or may attend the Morning and/or After Care program.

Child Health Report (You have one month to turn in this form.)
Children in Grades K-8 do not need the Child Health Report form if their Immunization/Health paperwork is on file in the nurse’s office.
Pre-K students do need the Child Health Report filled out.
PLEASE COMPLETE AND SEND THE FIRST TWO FORMS WITH YOUR CHILD.
NO CHILD WILL BE ABLE TO ATTEND WITHOUT THESE DOCUMENTS.
The Morning & After School Care programs will be billed to those that use it at the end of every month.
PLEASE NOTE: If your payment is more than 30 days past due, you cannot use the Morning/After Care service until it is paid. If your school tuition is more than 60 days past due, you cannot use the Morning/After Care service until it is paid.
The Morning and After School Program fee schedule is as follows:$8.00 an hour for one child
$9.00 an hour for two children
$10.00 an hour for three or more
If the child is picked up past a half hour, you will be charged for an hour. (Example: Our program begins at 2:30, pick up your child at 3:20 and pay $8.00)
After 6:00 pm, an additional fee of $1.00 will be charged to parents every minute. The late fee will be assessed accordingly and part of your monthly bill if necessary.
Please use the main door for drop off and pick up. Parents will sign their child(ren) in for Morning Care and out for After Care. If someone other than a parent is picking up, they will need to show proper ID.
The program will be closed due to:
Problem with the school building
School closing due to weather
Emergency situations
NOTE: THERE WILL BE NO BEFORE SCHOOL CARE ON 2 HOUR DELAY DAYS & NO AFTER SCHOOL CARE ON EARLY DISMISSAL DAYS.
We ask that you also send a nutritious snack for your child(ren) each day for After School Care. No medication will be administered during the After School Care Program.

Games/puzzles
TV/videos (If your child will be bringing their own electronic game, must be rated E)
